Youth Transition Services

​LINC’s Youth Transition Services support young people with disabilities, ages 14–26, as they navigate the critical years between adolescence and adulthood. We focus on preparing youth for competitive, integrated employment and independent living, while strengthening community connections.

Our services are designed to support youth as they move through key transitions:

  • School to School: For students changing schools or entering new programs, we provide skill-building activities, self-advocacy training, and peer support to ease adjustment and maintain progress.

  • School to Post-Graduation: We help youth and families plan for life after high school by exploring career options, higher education, vocational training, and independent living skills.

  • Graduation to Adult Living: Whether moving into new living situations, building daily life skills, or connecting to adult services, LINC provides guidance and opportunities to practice independence.

  • Transition to Employment: We emphasize career readiness through job exploration, workplace skill development, mentorship, and connections with employers who value inclusive, competitive employment.

Through in-person events, individualized planning, and community-based experiences, LINC meets youth and families where they are. Activities are developmentally appropriate, empowering young people to build confidence, determine preferences and personal strengths, gain practical skills, and engage fully in their communities.

By addressing every stage of transition, LINC ensures youth are not only prepared for what comes next but also supported in achieving long-term success, independence, and belonging.​

Cluster Club

Runs November-May

Cluster Club empowers participants with disabilities (ages 14–26)  to explore career clusters, develop practical life skills, and take ownership of their next steps — at a pace and style that fits their goals, needs, and lived experiences. 

Connecting participants with local employers, mentors, and peers to foster a supportive network that encourages independence and informed decision-making, Cluster Club helps young adults take meaningful steps toward their future — one experience at a time.

Next Bite

Runs March-September

Next Bite is an inclusive cooking club for teens and young adults (ages 14–26) with disabilities. We gather around the kitchen to learn meal prep and cooking skills in the spirit of a family-style meal. From trying out different appliances to building a personal recipe book, members gain the tools and confidence to prepare delicious dishes and foster independence. Whether we’re chopping, stirring, or sharing a meal together, Next Bite makes cooking fun, social, and empowering — one bite at a time.

Trail Mix

Runs June-September

Trail mix is an inclusive outdoor adventure club for teens and young adults (ages 14–26) with disabilities. We mix hiking, water sports, games, and nature-based activities with a focus on fun, friendship, and healthy living. Whether we’re exploring local trails, trying new recreation skills, or just enjoying fresh air together, Trail Mix helps every member build confidence, discover new hobbies, and create lasting memories — all while celebrating that the outdoors is for everyone.
